Summary

According to the abstract, lung tissue was prospectively collected from 15 CLAD patients undergoing retransplantation or autopsy and cultured with or without lipopolysaccharide or CD40L. The culture supernatant and a tissue eluate were tested for donor-specific antibodies (DSA, by Luminex) and non-HLA antibodies (by antigen microarray). Supernatant from all 4 patients who had serum DSA at lung isolation was positive for DSA, with higher levels after CD40L stimulation, but only 2 of these had detectable DSA in lung eluate. Non-HLA antibody levels in CD40L-stimulated supernatant correlated with those in lung eluate but not with serum. Flow cytometry showed higher frequencies of activated lung B cells, and immunofluorescence showed more IgG-positive plasma cells and greater IL-21 expression in lymphoid aggregates with local antibodies. The authors conclude that DSA and non-HLA antibodies can be produced within activated B cell-rich lung allografts. The study is a small tissue-based laboratory study and does not show a clinical effect. The authors include Tereza Martinu, Stephen Juvet and Shaf Keshavjee.
