Summary
This review evaluates lymphocyte-depleting and immunomodulating therapies for progressive CLAD beyond usual maintenance immunosuppression: anti-thymocyte globulin, alemtuzumab, methotrexate, cyclophosphamide, total lymphoid irradiation and extracorporeal photopheresis. The authors state that most interventions are only temporarily effective, and that, weighing efficacy and side effects, photopheresis, anti-thymocyte globulin and total lymphoid irradiation currently offer the best options. The significance statement notes that interpretation of most results is hampered by the lack of randomised controlled trials.
