Bronchoalveolar lavage cytokine-based risk stratification of clinically-stable lung transplant recipients with undefined rejection: Further insights from a follow-up investigation

Summary
This follow-up study included adult first bilateral lung recipients transplanted 2010 to 2017 who were clinically stable (no 10 percent or greater FEV1 drop) and had inadequate (grade AX) transbronchial biopsies in the first year. Seven proteins (IL6, S100A8, IL10, TNF-receptor-1, IL1 alpha, pentraxin 3 and CXCL10) were measured in lavage by multiplex bead assay. Among 107 patients, with biopsy at a median of 188 days after transplant, CXCL10 was significantly associated with CLAD, while IL6, S100A8, pentraxin 3, TNF-receptor-1 and IL10 were associated with death or retransplant (p below 0.05). The authors suggest such markers may identify patients needing closer monitoring. It is a retrospective association study and does not show cause and effect.
