Summary
The abstract describes the background to the proposal. Chronic rejection is described as the major cause of late death after lung transplantation. BOS was defined to describe an obstructive loss of FEV1. A restrictive form of chronic rejection had been described that does not fit the strict definition of BOS, so the term CLAD was introduced to cover all forms of graft dysfunction, although CLAD had not yet been defined. The authors propose a definition of CLAD and a flow chart intended to help recognise the different phenotypes. The abstract also mentions that some patients with BOS may respond to azithromycin with an improvement in FEV1 of more than 10 percent.
